Yes, a prescription is required for all compounded medications.
Only a USA licensed physician, physician's assistant, or nurse practitioner can write a compounded prescription.
Your doctor can send your prescription to the pharmacy via fax, phone, or electronically. If local, you can bring it in, or you may also mail us your original prescription.
We don't bill insurance directly, however, we would be happy to provide a Universal Claim Form to help you get reimbursed.
We ship anywhere we are licensed by either US Mail, Priority Mail, or FedEx.
